Hemodynamic studies in cardiogenic shock. Treatment with isoproterenol and metaraminol.

چکیده
In 14 subiects with severe cardiogenic shock the characteristic findings before treatment were hypotension, low cardiac output, and increased central venous pressure, with normal values for total peripheral resistance. Oliguria, lactic acidosis, hypocapnia. and hypoxemia also were present. These changes were interpreted as reflecting acute heart failure in association with a peripheral vascular response which was inadequate to maintain normal blood pressure. Isoproterenol caused a decrease in venous pressure and an increase in cardiac output. In most cases this was sufficient to increase blood pressure, despite slight reductions in peripheral vascular resistance. In a few cases, however, the inotropic effect of the drug was so small that the reduction in vascular resistance caused a further fall in arterial pressure. Metaraminol caused elevation of

Acute Hemodynamic Effects of Dopamine
The hemodynamic effects of dopamine were studied in 62 patients with clinical shock. In 36 patients with infection dopamine increased mean arterial pressure (MAP) 30%, and cardiac output (CO) 37%. Urine flow (UF) increased from 0.5 ml/min to 1.6 ml/min. Norepinephrine (NE) in 26 patients resulted in a higher MAP, lower CO, and similar UF. Isoproterenol (Isp) in 19 patients resulted in a lower M...
متن کاملSuccessful Treatment of Aluminium Phosphide Poisoning: A Case Report
Aluminium phosphide (AlP) is used to kill rodents in grain storage. It produces phosphine gas which is toxic for mitochondria. The exact mechanism of action is unclear, but phosphine is thought to produce toxicity by blocking cytochrome-c oxidase, which inhibits oxidative phosphorylation and result in cell death. This poisoning has a high mortality, and survival is unlikely if more than 1.5 g i...
متن کاملSearching for an ideal hemodynamic marker to predict short-term outcome in cardiogenic shock
Cardiogenic shock is a lethal condition. Physicians are searching for hemodynamic markers which could help risk-stratification of patients in this picture. Torgersen and coworkers present an hourly time integral of the cardiac power index and cardiac index drops to predict outcomes in the setting of cardiogenic shock. Continuous monitoring of hemodynamic markers may have a role in prediction of...
متن کاملHemodynamic Analysis of Pediatric Septic Shock and Cardiogenic Shock Using Transpulmonary Thermodilution
Septic shock and cardiogenic shock are the two most common types of shock in children admitted to pediatric intensive care units (PICUs). The aim of the study was to investigate which hemodynamic variables were associated with mortality in children with shock. We retrospectively analyzed 50 children with shock (37 septic shock cases and 13 cardiogenic shock cases) in the PICU and monitored thei...
متن کاملHemodynamic spectrum of myocardial infarction and cardiogenic shock. A conceptual model.
Despite the recent accumulation of a large hemodynamic data base describing myocardial infarction and cardiogenic shock, precise characterization of patient subsets has been elusive. This paper represents an attempt to identify the major factors contributing to this wide hemodynamic spectrum, and their interrelation using a theoretical model based upon currently emerging concepts of this diseas...
